Bulsai is a device that tackles a significant challenge faced by millions of diabetics: locating healthy tissue for injections to improve insulin absorption and avoid complications like lipohypertrophy (LH). LH occurs when repeated injections in the same area cause scar tissue, cysts, and fatty deposits, blocking insulin absorption and causing blood sugar swings. Bulsai uses portable ultrasound and AI to find the best injection sites.
Handheld and portable—about the size of a large marker—Bulsai is designed for simplicity. It uses a red/green light system to guide users to the best injection areas and provides the option to mark these spots with erasable ink stamps or small marks for accuracy. Beyond site identification, the device also:
• Helps find the right depth for leaner patients, ensuring injections reach muscle tissue to reduce pain and improve absorption.
• Scans injection sites after use to check if insulin has been properly absorbed.
• Uses ultrasound to improve insulin absorption.
Bulsai addresses a critical issue for 8.3 million insulin-dependent diabetics in the U.S., where poor injection site selection can reduce insulin efficacy, causing hyperglycemia, hypoglycemia, or overdoses.
The impact of Bulsai is significant. Each year, over 16 million emergency room visits are linked to diabetes, many due to dosing errors. Around 70% of insulin-dependent diabetics inject into unhealthy tissue, reducing insulin effectiveness and causing glucose fluctuations. By promoting proper injection practices, Bulsai lowers hospitalizations, improves daily health, and provides long-term benefits.
Bulsai, a patent-pending innovation, has proven the value of ultrasound technology in diabetes care through a two-month feasibility study. It is the only device of its kind, combining portable ultrasound and machine learning for a transformative solution. Partnerships with Genesis Innovation Group (medical device development and commercialization), Tekna (prototype design and market research), and A Wright Path (FDA clearance and regulatory guidance) are expediting its development.
One year ago, Bulsai also partnered with Startup Health to help propel its market entry, but as of today, the device has not yet been launched. The Bulsai team aims to create a prototype and secure additional funding for regulatory processes, market research, and manufacturing. While the device is particularly beneficial for older patients with advanced scarring or LH, it is equally valuable for younger patients by helping them avoid injection-related complications from the outset.
