About the solution
The idea for Telos Care was born from a personal challenge. Ana and Guilherme, siblings, shared the responsibility of accompanying their grandparents to doctor visits and exams. But when they had to take turns, even missing a single appointment meant losing critical context about their grandparents’ health, making it difficult to communicate effectively with healthcare professionals. Simple questions about symptoms and exam history became hard to answer. This experience revealed a common problem: the constant loss of important health information between patients, families and healthcare providers.
Even though healthcare systems are becoming more digital, a crucial gap remains. Patients still lack simple, effective tools to actively manage and communicate their health information. Remembering symptoms, medications, and medical history is key during short consultations, as it helps clinicians deliver better treatment, but it is difficult, especially when caring for someone else. On top of this, health data is often scattered across different providers and platforms, creating inefficiencies and increasing the risk of duplicated exams and overmedication.
Telos Care was created to close this gap. It is a patient-centered mobile app that organizes fragmented health information into structured, event-based medical journeys. Appointments, exams, and treatments can be added manually or captured with AI from photos and screenshots, making data entry fast and intuitive. These events can be linked and enriched with notes, symptoms, questions for doctors, or follow-up actions. Instead of isolated records, the app builds a living, connected journey of a person’s health that is easy to understand, track and share.
Sharing is at the heart of the solution. With Telos Care, patients can securely involve family members and informal caregivers, ensuring everyone has access to the same up-to-date information. This collaborative approach makes care less overwhelming and more transparent while also strengthening communication with healthcare professionals through better-informed interactions.
By centralizing and contextualizing health information, Telos Care empowers patients – especially those living with chronic conditions – and their informal caregivers to take an active role in care management.
