Health-related information and calls-to-action are often delivered in a one-size-fits-all manner, but patients have unique medical needs, demographics, motivations, barriers, and levels of health literacy. A personalized approach is critical to improving patient activation and health outcomes by delivering information in ways that are tailored to each individual's needs.
